WASHINGTON — Federal investigators found one safety violation, then another, and then another, while reviewing last year’s fatal train crash in Chester.
Ultimately, investigators from the National Transportation Safety Board (NTSB) found 20 separate safety lapses that contributed to the April 3, 2016, collision between a train and a backhoe on an adjacent track. Two Amtrak workers died and 39 people were injured.
