Australia – ‘There was blood everywhere’: inquiry to find out why train failed to stop

Three separate regulators will investigate why a passenger train careered into a buffer stop at Richmond station, sending more than a dozen commuters to hospital and raising questions about the brakes system on the new Waratah train.

The all stations from Leppington to Richmond service took off from its penultimate stop at East Richmond on Monday morning with its driver looking towards the terminal and its passengers oblivious to what was about to occur.
